Nominations are now open and will close on December 14th.

Nominations submitted after December 14th will not be considered.                                                                                                                                                                 
 
BASIC REQUIREMENTS FOR ELIGIBLE NOMINATIONS

• Nominations must be submitted within the submission window to be considered eligible.
• All information provided in the nomination form must be true and complete.
• To be considered an eligible nomination and a qualified nominator, the person(s) submitting the nomination cannot live within the household [currently and/or have plans to live within the family household] of the family being nominated.
• Self-nominations are not accepted. Nomination forms will not be accepted if the person(s) submitting the nomination form is also the recipient or beneficiary in any tangible way.
• Person(s) considering or submitting a Nomination Form must ensure the confidentiality of the nomination process. Information about the nomination, including any prior knowledge communicated to the family, must remain undisclosed.
• Person(s) submitting a nomination must agree to further communication and exchange of information with Mendi Group immediately following the close of the nomination window. If the person(s) is unable to further communicate/exchange information regarding the family nominated, the family nomination may be disqualified.
• Employees of Mendi Group are ineligible to nominate or be nominated.

 
DETAILS TO CONSIDER

• All nominations must be submitted within the nomination window.
• All submissions will be carefully considered for the current year only.
• Please be as descriptive as possible.
• Due to the number of nominations, only 3 families will be chosen.
• Nominators will be contacted directly if their nominated family is chosen.
